Below are the steps you will need to follow to get the birth certificate.

1. Complete the four-page Birth Certificate Application
2. If you are not married to the father, complete the Affidavit of Paternity form.
3. Complete the Affidavit Forms. One for the Mother and the other for the Father
4. Include a photo copy of a picture ID (Drivers License or Passport)
5. Include a Rent/Mortgage receipt
6. Just incase, also include a utility bill.
